We work closely with patients, families and physicians to develop individualized care plans to best meet the needs of each patient
Services include: Skilled nursing, skilled therapy, medical social work and home health aide services

What You’ll Do
Responsible for assessment and planning patient care in conjunction with other disciplines in home care. Provides social work service up to and including discharge for patients requiring those services. Assists client in obtaining available community, State or Federal services.
Requirements:
Bachelor's degree in Social Work.
Current Kansas Social Worker License
Minimum 1 year Social Work experience in a Health Care setting.
Auto liability insurance.
Clean driving record.
Current driver's license.
Must be able to successfully pass KDADS criminal record check upon hire.

Preferred Requirements:
Current Clinical Social Worker licensure in state of practice.
Continual Education:
AHA BLS for Healthcare Providers Number of Days: 30
