Salary £36,363 - £40,777 | Full time | Permanent | Intermediate Care Team | County Wide

We are Hiring!

We are looking for a motivated and skilled Social Worker to join our Intermediate Care Team, supporting adults to maximise independence and achieve positive outcomes following hospital discharge or during periods of recovery.

This is a fast-paced, strengths-based role where you will work with people for a short period of time, helping them regain skills, confidence and stability to remain living safely in the community.

The Intermediate Care Team is a multi-disciplinary service, working closely with health colleagues including occupational therapists, physiotherapists, nurses and reablement staff. Together, we provide timely, person-centred interventions that prevent unnecessary hospital admissions and support safe discharge.

Key Responsibilities

  • Carry out strengths-based assessments and develop short-term support plans
  • Undertake Care Act assessments, reviews and Mental Capacity Assessments
  • Work collaboratively with health professionals, families, and carers
  • Support hospital discharge planning and prevent delayed transfers of care
  • Promote independence and reduce long-term care needs wherever possible
  • Manage a dynamic caseload within tight timescales
  • Complete accurate, timely and high-quality recording
