
Elite Home Health & Hospice is seeking a compassionate and dedicated Medical Social Worker (BSW or MSW) to join our interdisciplinary team serving patients across both our Home Health and Hospice service lines. The Medical Social Worker assists patients and families in addressing social, emotional, psychosocial, financial, and environmental concerns related to illness, disability, end-of-life care, and recovery. Working as part of an interdisciplinary team, the Social Worker helps identify barriers to care, connects patients with community resources, and provides counseling and support throughout their healthcare journey.
The ideal candidate is compassionate, resourceful, and patient-focused. They build trusting relationships with patients and families, communicate effectively with healthcare professionals, and demonstrate a commitment to improving quality of life through advocacy, education, and support.

Pay: $26-$31 per hour
