Job Description

Responsibilities:

  • Assesses the environment of the home and community to identify elements which impact negatively on the patient/family ability to function.
  • Provides direct short-term counseling services to patient/family or makes appropriate community referrals.
  • Teaches coping skills to patients/families to assist them in their adjustment to the life changes associated with illness/disability. 
  • Participates in conferencing and planning with other disciplines caring for the patient. 
  • Prepares clinical progress notes and documents in the patients record, as indicated. 
  • Available for planned social work consultation on patient care with staff, as needed. 
  • Participates in evaluating overall position performance, goal setting and achievement, and performance improvement plan.

Qualifications:

  • New York State Social Work License
  • 2 years of social work experience
  • Master’s Degree
