GENERAL DESCRIPTION:
The Social Worker is the Client Care Coordinator. Client Care Coordinators improve access to mental health services in the Family with Children shelter system and assist families that are homeless as they navigate multiple systems and cope with the stressors and anxiety induced by homelessness. The Social Worker provides clinical guidance to onsite social service/shelter staff.  The Social Worker also serves as the liaison between the families and the Director.
The Social Worker is classified as an essential worker, and as such accepts responsibility for the completion of their critical services to participants, on site, even during times of emergency. These include, but are not limited to, emergencies caused by weather, nationwide pandemics, civil unrest, and domestic threat.
MAJOR DUTIES AND RESPONSIBLITIES:
- Provides clinical guidance and trainings to social service staff/shelter staff.
- Completes biopsychosocial assessments and mental health and substance use screenings.
- Conducts case conferences; resolves participant issues.
- Constructs service plans, program notes, and other documentation and ensures compliance with funder, regulatory and LSSNY requirements. Collaborates with ACS where there is child welfare involvement.
- Provides psychoeducation to families and meets with them quarterly to discuss progress.
- Conducts self-audits of participant records.
- Participates in regular staff meetings.
- Serves as member of incident reporting committee and strategizes follow-up along with other members of social service team.
- Conducts participant assessment and crisis intervention. May provide short term counseling (not psychotherapy).
- Makes referrals and appropriate linkages.
- Holds participant groups and workshops.
- Serves as on-site liaison for participants.
- Develops and maintains relationships with community resources.
- Assists with other duties assigned by the Shelter Director or Team Leader and other functions related to the program.
- Adheres to confidentiality protocols outlined by the agency.
- Represents Lutheran Social Service of New York’s mission and values; upholds agency Code of Conduct and other policies and procedures.
- Demonstrates commitment to social justice and promotes equity by identifying and removing barriers, facilitating learning, cultivating community, and empowering others.

QualificationsEDUCATION:
- Licensed Master of Social Work degree
EXPERIENCE:
- Minimum of 2 years’ experience in the field of social services.
- Working knowledge of issues related to or experience working with families, homelessness, HIV, substance abuse, mental illness, entitlements and domestic violence.

SKILLS & ABILITIES:
- Excellent clinical skills.
- Strong writing and speaking skills.
- Highly organized and detail oriented.
- Strong time management; able to handle multiple tasks and assignments at once.
- Working knowledge of Microsoft Office (Word and Excel).
- Bilingual a plus, preferably in Spanish.
- Knowledge of CARES system a plus.

OTHER REQUIREMENTS:
- Crisis intervention skills.
- Familiarity with Health Insurance Portability and Accountability Act (HIPAA).
