The Social Worker provides trauma-informed, family centered support to strengthen parent-child relationships promoting healthy youth development through respite services. Including delivering evidence-based interventions and conducting in-home sessions across all five boroughs to help families build protective factors and achieve lasting stability

Responsibilities

The Social Worker provides trauma-informed, family centered support to strengthen parent-child relationships promoting healthy youth development through respite services. Including delivering evidence-based interventions and conducting in-home sessions across all five boroughs to help families build protective factors and achieve lasting stability

Responsibilities:

  • Lead family-centered service planning and team coordination - Serve as the team lead in developing individualized treatment plans and conducting referral assessments in collaboration with families and interdisciplinary staff. Ensure cohesive, goal-driven service delivery aligned with ACS
  • Deliver GenPMTO and best practice interventions - Facilitate GenPMTO training and implement evidence-based strategies to strengthen parenting skills, enhance family functioning, and support positive outcomes during and after respite stays.
  • Conduct in-home sessions across all boroughs - Provide home visits and direct support to families and youth in all five boroughs to promote engagement, stability, and continuity of care.
  • Coordinate crisis prevention and stabilization - Respond to family crises with safety planning and emergency support, working closely with ACS and internal teams to stabilize the home environment.
  • Engage youth and support transitions - Work directly with youth in collaboration with the multidisciplinary team to address behavioral challenges and support emotional development, including planning and facilitating transitions from respite homes to origin homes.
  • Connect families to community resources - Research, organize, and share information on local services, and link families to mental health, substance abuse, domestic violence, and other supports to meet their goals
  • Maintain documentation and track outcomes - Complete all required documentation in CNNX, Evolv, and Promis databases, and maintain data systems to track KPIs and analyze trends that inform service delivery and program improvement.
  • Participate in training and administrative support - Engage in ongoing professional development, model training, and administrative tasks to enhance program delivery and meet evolving family and child needs.

Qualifications

  • Master’s Degree in Social Work, Mental Health, Psychology or related Field. Bilingual Preferred language skills a plus.
  • 2+ years’ experience in working with or providing services to at risk children and families in Child Welfare, and experience in crisis intervention skills. Knowledge of infant and child and adolescent development
  • Experience and willingness to work with the culturally diverse populations that are present among the program's target population
  • Open to reflective practice (i.e. has capacity for introspection, communicates awareness of self in relation to others, recognizes value of supervision)
  • Ability to identify family strengths and foster self-sufficiency and independence in families
  • Strong communication skills (verbal and written)
  • Flexibility with schedule as program will need evening and weekend support. This is a 24/7 on call program for emergencies that arise during after-hours.
  • Driver License preferred but not required

Education Required

Masters’s Degree from a U.S. accredited educational institution or Credential Evaluation for a non-U.S. educational institution

Salary Range:

$63,430.49/Annual + $3,500 Bilingual Additive (if applicable) + $1,000 – Provisional Licensure Additive (as applicable) + $4,000 – Full Licensure Additive (as applicable)
