The Social Worker reports to the Clinical Services Manager, Emergency, with professional accountability to the practice leader for social work. The successful candidate will be part of a multi-disciplinary team responsible for the care of patients and families throughout the continuum of life, dealing with such issues as bereavement (including early pregnancy loss), end of life issues, domestic violence, child abuse, difficult family situations, facilitated discharge and staff incident debriefing.

Accountabilities:

  • Works collaboratively with the allied health team in coordinating patient plan of care
  • Represents social work professional practice in various Emergency Department related meetings
  • Participates in yearly strategic planning initiatives and corporate professional development

Qualifications

  • Masters degree in social work
  • Registration with the OCSWSSW
  • Minimum of three to five years social work experience in an emergency department
  • Previous work history of dealing with one or more of the following client types/issues: elderly, homeless, domestic violence, paediatric, substance abuse and bereavement
  • Knowledgeable in community resources
  • Knowledge and experience working with relevant legislation (i.e. SDM Act, POA, consent and capacity, etc.)
  • Knowledge of the psychosocial impact of chronic illness on the person and the family
  • Excellent communication and interpersonal skills
  • Ability to work as part of a multi-disciplinary team
  • May be required to travel between and work at all Osler sites
  • Excellent organization skills and ability to set priorities as required
  • Good critical thinking and problem-solving skills required
  • Demonstrated excellent organizational, interpersonal and communication skills
  • Must be able to work independently and interdependently within multidisciplinary treatment team
  • Computer proficiency with Microsoft Word, PowerPoint and Outlook as well as Meditech
  • Excellent attendance, punctuality and work record
  • Must demonstrate Osler's Values of Respect, Excellence, Service, Compassion, Innovation and Collaboration

Additional Information

Hours: Currently Days/Evenings 8 hours shifts, including weekends and statutory holidays (subject to change in accordance with operational requirements)

SALARY:

Minimum: $45.58 per hour

Maximum: $59.09 per hour
