Our client, based in Solihull, is seeking an experienced Social Worker to join their busy Discharge to Assess Team. This is an exciting opportunity for a skilled Adult Social Care practitioner to support individuals transitioning from hospital settings back into the community, ensuring safe, timely, and person-centred discharges.
Working within a fast-paced environment, you will undertake Care Act assessments, support planning, mental capacity assessments, and safeguarding work to ensure people receive the right support at the right time. The successful candidate will be confident managing their own caseload, balancing competing priorities, and working independently across a range of complex situations. A strengths-based approach and commitment to promoting independence and wellbeing will be central to your practice.
