About Us – Dialysis
Freeman Nephrology and Dialysis consists of a multidisciplinary team-working together to provide compassionate care for patients with various kidney-related diseases
Outpatient dialysis services are provided at Freeman Hospital East and Webb City Neighborhood Care
Freeman Outpatient Dialysis Center in Webb City features 16 comfortable chairs, state-of-the-art equipment, innovative technology and a safe, private environment for patients
Partner of Saint Luke’s Health System Kidney Transplant Program
What You’ll Do
Functions as a member of the multi-disciplinary health care team to assess and plan for patients to leave the acute care and/ or acute rehab setting. Responsible for completing psychosocial assessments, coordinating discharge plans, needs, and arrangements for patients, and for providing information, education, and linkage with appropriate community resources and services.
Requirements
Current Missouri License for LMSW.
Master's degree in Social Work with a specialization in clinical practice from a school of social work accredited by the Council on Social Work Education OR Served at least 2 years as a social worker, 1 year of which was in a dialysis unit or transplantation program prior to Sept 1, 1976, and has established a consultative relationship with a social worker who qualifies under 494.140 (d)(1).
Preferred Requirements
Current Missouri License for LCSW.
