
Prepares the patient and/or patient’s family to transition from the hospital to the next appropriate care setting based on their individual needs. Applies their understanding of medical conditions, insurance coverage and how physical/psychosocial may affect the patient once a patient leaves the medical setting to plan and initiate an individualized discharge plan. Provide clinical information to payers and/or post-acute providers in order to obtain authorizations for hospitalized patients
Education required Bachelor of Social Work (unless MSW required for licensing) from an accredited institution
Education preferred: Masters of Social Work from an accredited institution preferred
Experience: 3+ years acute care hospital experience preferred
License/Certificates: Licensed Medical Social Worker (i.e. LSW, LCSW, ACSW, etc) in Alabama OR Licensed in Alabama within 6 months of employment
