This position is located in Community Long Term Care (CLTC), Spartanburg County.
This is an in-office role and not a telecommute or remote position.
Are you the One? We are looking for a Social Worker (Case Manager II), who works under general supervision of CLTC Area Administrator. Serves as Case Manager Il. Assists in the implementation of a system of case management for Medicaid eligible applicants and enrolled participants. Assists other CLTC offices with essential job functions according to program needs. Trains workers and performs assigned quality assurance activities and compliance activities as directed. Travel to location of assigned cases necessary for this position. Maintains a thorough working knowledge of all aspects of CLTC and related systems: programs, policies, and procedures, including but not limited to waivers, Phoenix and Care Call.

A bachelor's degree in social work or social welfare program accredited by the Council on Social Work Education, or from a social work or social welfare program offered by a regionally accredited institution, plus one (l) year of experience as a professional social worker.
NECESSARY SPECIAL REQUIREMENT: Current licensure by the SC Board of Social Work Examiners. The ability to multi-task within time frames and prioritize cases utilizing effective time management skills. Effective time management skills and computer proficiency. Must be able to perform web-based software functions (key participant data, run reports, update documents, etc.)
