Day shift
Provides advanced clinical and psychosocial support to high-risk, medically complex patients. Responsible for assessment, care coordination, discharge planning, crisis intervention, and addressing social determinants of health. Assists in transitional care management as needed. Promote patient outcomes, supporting value-based care, and reduction in avoidable readmissions.
Minimum Education, Training, and Experience:
Required Qualifications:
A. Masters or Doctoral Degree in Social Work
B. Certified Social Worker certificate preferred.
C. Must show proficiency in completing assessments, treatment planning, aftercare planning and crisis management.
D. Communication skills must be exceptional with the ability to interact professionally with professionals of multiple disciplines.
E. Minimum of 5 years clinical case management experience.
