Compensation: $51,000 - $61,200 / year

Schedule Exempt, min 40 Hours per week | Monday - Friday | 8am - 4:30pm

What You’ll Do:

As a Social Worker (BSW) in our PACE program, you’ll empower participants to live their best lives—supporting emotional well-being, guiding care transitions, and connecting them to valuable community resources. You’ll be a champion for independence and quality of life, working alongside a dynamic interdisciplinary team to create meaningful, person-centered care every day.

Key Responsibilities

Case Management & Care Coordination:
• Manage and coordinate care plans, working closely with healthcare professionals, families, and community organizations.
• Facilitate participant transitions of care, ensuring compliance with policies and smooth handoffs between care settings.

Participant Support & Advocacy:
• Conduct wellness checks to assess the health and well-being of participants in various settings.
• Assist participants in accessing community resources such as housing, healthcare, and financial assistance.

Screening & Eligibility:

• Assist in assessing financial eligibility for programs, including supporting applications and required documentation.
• Perform screenings to identify participant needs and make appropriate recommendations.

Collaboration & Communication:
• Maintain ongoing communication with participants, families, facilities, and external agencies to coordinate care and resolve issues.

Skills, Knowledge and Expertise

Education & Certification

• Must possess a Bachelor’s degree in Social Work (BSW) from a program accredited by the Council on Social Work Education (CSWE).
• Active LLBSW/LBSW license in the state of Michigan.
• Minimum of one year of experience working with a frail or elderly population (preferred).
• Must possess a current, valid driver’s license and maintain appropriate auto insurance.
• Current CPR certification, or willingness to obtain within 6 months of employment with LifeCircles.
• Strong desire to work in a team-based model of care.
• Excellent verbal and written communication skills.
• Highly organized with strong time management abilities.
• Proficient in Microsoft Office Suite (Excel, Outlook, Word) and electronic medical records systems.
• Demonstrates professionalism, critical thinking, creativity, flexibility, and strong problem-solving skills.
• Maintains high ethical standards and a strong sense of integrity.
• Comfortable working independently and managing self-directed daily responsibilities.
