Job Description


Reporting to the Manager Department of Integrated Discharge Services, the registered social worker will work to complete weekly mandatory ALC reports for the hospital as directed by the province.

QUALIFICATIONS:
Bachelor of Social Work degree from an accredited University required
Master’s of Social Work degree from an accredited University preferred
Current registration with the Ontario College of Social Workers and Social Service Workers
Minimum of 5 years frontline social work in a hospital setting preferred
Experience utilizing computer software such as word, outlook, excel, power point, WTIS ALC IS, and other computer systems to view data, create and submit reports
Experience working within a hospital setting preferred
Knowledge of community resources preferred
Strong communication skills required
Ability to maintain consistency with the mission and values of St. Joseph’s Healthcare Hamilton
Strong attention to detail, multi-tasking and prioritizing when responding to requests for reports
Ability to shift attention and priorities based on demands of department and hospital

RESPONSIBILITIES:
Maintains confidentiality and release of information in accordance with hospital policy at all times
Obligation to review and commit to applicable departmental and hospital policies and procedures at all times
Complete mandatory ALC reporting requirements for the hospital to be submitted to internal and external stakeholders
Complete ad hoc reports as requested by various internal stakeholders related to the interpretation and understanding of the hospital’s ALC rates
Assist social workers and other hospital staff as requested with the understanding and application of ALC to specific patients or patient populations
Keep up to date regarding ALC provincial definitions and directions and work with the social work team to make recommendations to the hospital to meet provincial ALC rates
Identify systemic barriers within the health care system that keeps patients from being in the right place for the right care at the right time"
Advocate where appropriate for the development, provision and expansion of required services to close gaps in patient care
Ensure ALC data is accurate and data has integrity
Provide education to new social work staff on the clinical application of ALC definitions and the process for submitting accurate data
Educate and Train staff to complete ALC reports to ensure adequate coverage for mandatory weekly reports as required
Demonstrate leadership in discharge planning. Help social workers to problem solve difficult cases with respect to ALC designation and the identification of appropriate discharge locations
Assist with clinical coverage as requested
Perform other duties as assigned
