Social Worker - Adult Sickle Cell - Days

We are seeking a Social Worker to join our outpatient Adult Sickle Cell multidisciplinary care team. In this role, you will conduct comprehensive biopsychosocial assessments and develop individualized plans of care for patients and families managing chronic illness and complex psychosocial needs. You will provide counseling, crisis intervention, advocacy, psychoeducation, and ongoing psychosocial support to help patients and families navigate the healthcare system, access community resources, strengthen coping skills, and enhance their overall quality of life. Working closely with physicians, nurses, social workers, and other members of the interdisciplinary team, you will facilitate family meetings, support educational and patient-focused programs, and serve as a trusted resource throughout the continuum of care. You will also maintain accurate electronic medical records and provide timely written and verbal communication to the Sickle Cell treatment team.
This is a full-time position, Monday-Friday, 9am - 5pm at NewYork-Presbyterian/Columbia Irving Medical Center.
Preferred Criteria
Outpatient health/mental health experience
Experience working successfully as part of an interdisciplinary care team
Experience supporting patients with chronic illness and complex psychosocial needs
Fluent in Spanish
Required Criteria
New York State License as a Licensed Masters Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)
Strong assessment, counseling, crisis intervention, advocacy, care coordination, communication, and collaboration skills
Salary Range:
$43.86-$58.73/Hourly
