Job Description

Position Summary

This position is funded by a grant for a duration of time as defined by the grantor.

This position will provide intensive, comprehensive case management wrap-around services to individuals and families under the direction of the Community Services Block Grant (CSBG) Supervisor.

Pay Grade: 48

Salary Commensurate with Experience and Education

Minimum Qualifications

  • Three (3) years full-time work experience in social work, counseling, or a closely related field; AND
  • Bachelor's degree from an accredited college or university in social work, counseling, or closely related field;
  • An equivalent combination of related education and/or experience.
  • Master's degree in social work preferred.
  • PROOF OF EDUCATION, TRAINING, AND/OR EXPERIENCE IS REQUIRED.

Duties and Responsibilities

  • Assesses the social and emotional needs of low-income households to develop service plans achieving stability and financial independence.
  • Conducts thorough interviews to share program criteria and to assess program commitment readiness.
  • Reviews and executes agency/customer impact with household depicting roles and responsibilities of each party.
  • Assists in the program orientation of participating customers.
  • Creates an in-depth, customer-focused and customer-driven case treatment plan with identified goals, objectives and metrics for the purpose attaining identified outcomes specific to each household member.
  • Ensures that case plan aligns with 2Gen Model, Transition to Success Model, Whole Family Approach- depending on the customer's needs.
  • Advocates and/or mediates on behalf of the household, making necessary local provider referrals and ensuring warm hand-off and successful outcomes.
  • May conduct site/home visits to assess needs and to determine growth and progress.
  • Ensures timely and accurate data entry and case notes into required service databases, as well as maintaining accurate and organized client files.
  • Tracks and follows up on all referrals made on behalf of the customer and ensure outcomes are documented, timely, in the database.
  • Participates in quality improvement processes.
  • Assists in the delivery of all CSBG-related services to ensure cross-braiding of funds and ample staff coverage.
  • Attends meetings and training sessions as required or instructed.
  • Performs other related duties as required or directed.

KSAs

  • Knowledge of the principles and techniques of counseling.
  • Ability to effectively communicate both orally and written.
  • Knowledge of local resources available to assist customers such as, but not limited to, social security, medical insurances, financial management, job development, employment and educational opportunities, as well as other social services.
  • Proficient knowledge in Microsoft Office programs.
  • Ability to plan, organize and maintain an even flow of work.
  • Proficient knowledge in the use of databases, including input and reporting. Bilingual applicants highly desired.
  • Ability to engage and diffuse crisis situations.
  • Ability to work effectively with individuals and families who are income challenged and whose economic status is considered low-income.
  • Ability to practice from a perspective of compassion and build positive rapport with customers when providing services.
  • Ability to communicate effectively with customers, co-workers, and community partners.

All employees hired after September 1, 1986, must be residents of Shelby County and shall continue to reside in the County as a condition of their employment. The residency requirement shall not apply to certain public safety/civil service employees. This means exemption from this policy is in effect for the following departments and positions: (1) Sheriff Deputy Patrol Officers, Deputy Jailers and Dispatchers, employed by the Sheriff Department, (2) Correction Officers employed by the Division of Corrections, (3) Firefighters, Paramedics and Dispatchers employed by the Shelby County Fire Department. The residency exemption for public safety/civil service employees does not include appointed positions with the Sheriff’s Office, Division of Corrections, or the Fire Department.
