Social Worker for Skilled Nursing

We are seeking a compassionate and dedicated Social Worker to join our skilled nursing team. Full-time exempt position, excellent benefit package after 60 days including medical, dental and vision. Paid vacation and sick time, holiday pay, daily meal allowance, 403b match up to 6%, and other benefits.

Key Responsibilities

• Conduct psychosocial assessments of residents to identify their needs, preferences, and potential barriers to care
• Develop and implement individualized care plans in collaboration with residents, families, and healthcare teams
• Provide emotional support to residents and their families to help them cope with health challenges and transitions
• Coordinate with community resources, agencies, and support services to meet residents’ social and emotional needs
• Advocate for residents’ rights and preferences within the healthcare setting
• Document all interactions, assessments, and care plans accurately and in compliance with regulatory standards
• Participate in interdisciplinary team meetings to ensure comprehensive resident care
• Educate residents and families about available services, benefits, and community resources

Requirements

Skills and Qualifications:

• Bachelor’s degree in Social Work (BSW) or related field; Master’s degree preferred but not required
• Valid state licensure or certification as a Licensed Social Worker (LSW) or equivalent
• Proven experience working in a healthcare or skilled nursing environment experience preferred
• Strong interpersonal and communication skills, with the ability to build rapport with diverse populations
• Knowledge of healthcare regulations, Medicaid/Medicare policies, and community resources
• Ability to handle sensitive situations with empathy, professionalism, and discretion
• Excellent organizational and documentation skills
• Ability to work collaboratively within a multidisciplinary team
