Serving as the primary point of entry for community members, this role provides immediate emotional support, and psychosocial assessments for individuals and families seeking assistance. The position ensures structured intake, crisis intervention, and access to necessary services while upholding strict confidentiality, privacy, and statutory requirements in accordance with POPIA and professional standards. Through short-term engagement, the position evaluates psychosocial, financial, and care needs to determine eligibility and recommend appropriate internal or external service pathways . Working collaboratively with multidisciplinary teams, referral networks, and community stakeholders, the role facilitates seamless care coordination and administrative case management . Key Responsibilities Deliver immediate crisis intervention, trauma containment, emotional support, and safeguarding for callers and individuals in distress . Guide clients through available services, admissions processes, documentation requirements, and access pathways . Conduct comprehensive psychosocial, financial, and home-based care needs assessments through telephonic interviews, face-to-face engagements, home visits, and hospital visits . Evaluate referrals from telephone, email, website, families, professionals and internal divisions. Report and escalate at-risk matters with immediacy to appropriate authorities or external service providers . Facilitate referrals to suitable internal divisions and external community resources following full assessment . Verify, collate, and process supporting documentation to enable ongoing service delivery . Capture and maintain accurate electronic client records, intake documentation, formal assessment reports, and case notes . Maintain records of general enquiries, statistical reporting, and up-to-date community resource networks . Participate in professional supervision, performance reviews, staff meetings, and continuous professional development . Essential Requirements Bachelor’s Degree in Social Work, Psychology, or Counselling . Professional registration with the SACSSP or HPCSA . Minimum of 3 to 5 years of relevant experience . Proven experience conducting psychosocial, financial, and home-based care needs assessments . Demonstrated knowledge of crisis intervention, trauma containment, and safeguarding practices . Valid driver’s licence and willingness to perform home and hospital visits . Proficient in written and verbal communication, interpersonal engagement, and professional report writing . Demonstrated ability to maintain strict confidentiality in alignment with POPIA and relevant registration council guidelines . Desirable Requirements Strong problem-solving abilities, emotional resilience, and sound clinical judgement . Ability to work effectively both independently and within multidisciplinary teams . Required Software Skills Computer literacy and experience working with electronic case management systems. Work Arrangement On-site, with required home and hospital visits .
