Job Description

Job Title: Social Worker
Location: White Plains, New York
Schedule: Full-Time | Day Shift | Monday–Friday, 9:00 AM–5:00 PM
Weekend/Holiday Requirement: Alternating weekends and holidays
Hours: 75 hours per pay period
Pay Range: $37.45–$56.17 per hour ($73,019–$109,529 annually)

A leading healthcare organization is seeking a Social Worker to support patients throughout their hospitalization and transition of care.

The Social Worker will complete comprehensive psychosocial and discharge-planning assessments, identify patients at high risk for readmission or complex discharge needs, and coordinate appropriate services across the continuum of care. This role works closely with patients, families, physicians, nursing staff, case management, payers, and community resources to ensure safe and effective discharge plans.

The ideal candidate will hold a Master of Social Work degree and an active New York State LMSW or LCSW license.

Responsibilities

  • Complete comprehensive initial psychosocial and discharge-planning assessments upon admission
  • Screen patients using established tools to identify high-risk indicators and potential barriers to discharge
  • Reassess patient needs throughout the hospital stay
  • Develop and coordinate individualized discharge and transitional care plans
  • Arrange post-acute services, community resources, and follow-up care
  • Provide education and support to patients, families, and caregivers
  • Coordinate care across hospital departments and outside healthcare organizations
  • Collaborate with physicians, nurses, case managers, rehabilitation professionals, and other members of the interdisciplinary team
  • Communicate relevant patient information to appropriate healthcare team members
  • Assist with commercial payer and insurance-related discharge coordination when needed
  • Evaluate frequent-utilizer patients and help identify factors contributing to repeated hospital utilization
  • Support initiatives focused on reducing length of stay and preventing avoidable readmissions
  • Help reduce discharge delays and payer denials through proactive care coordination
  • Advocate for patient needs, preferences, safety, and access to appropriate resources
  • Maintain accurate and timely documentation in the electronic medical record
  • Participate in departmental performance-improvement and patient-satisfaction initiatives
  • Maintain compliance with hospital policies and applicable regulatory standards
  • Perform other related duties as assigned

Qualifications

Required

  • Master of Social Work degree
  • Current New York State license as one of the following:
    • Licensed Master Social Worker (LMSW), or
    • Licensed Clinical Social Worker (LCSW)
  • New York State PRI/SCREEN certification within 90 days of hire
  • Strong psychosocial assessment and discharge-planning skills
  • Ability to coordinate complex transitions of care
  • Experience working collaboratively with interdisciplinary healthcare teams
  • Strong written and verbal communication skills
  • Ability to educate and support patients, families, and caregivers
  • Proficiency with electronic medical records
  • Strong organizational, prioritization, and problem-solving abilities
  • Ability to work alternating weekends and holidays as required

Preferred

  • Two or more years of Social Work or Care Management experience in an acute-care hospital or related healthcare setting
  • Experience with complex discharge planning
  • Experience coordinating skilled nursing, rehabilitation, home health, hospice, or other post-acute services
  • Knowledge of insurance, commercial payer, and community-resource coordination
  • Experience with readmission-prevention and length-of-stay initiatives
