Priory Hospital Middleton St George has an opportunity for a Social Worker to join our team.
Priory Hospital Middleton St George, Darlington, County Durham is a specialist inpatient service for adults with mental health issues, complex needs and behaviours that challenge. It is accessible by public transport, with a local train station approximately 5 miles away and local bus services to Darlington.
What you'll be doing
The role of the Social Worker is to participate as an active member of the Multidisciplinary Team. The Social Worker is allocated to work across site and participates in ward rounds, risk assessments, care planning, discharge planning, decision-making and team meetings. In this role, you will play a key part in patient recovery, supporting the assessment and delivery of tailored treatment plans while ensuring dignity, respect, and high standards of care.
- To engage effectively with service users to assist with each person’s rehabilitation and recovery
- To provide a link with service users’ locality services and the Multi-Disciplinary Team
- To provide a link with service users’ social supports (carers, family members, friends) and the Multi-Disciplinary Team
- To act as Carer Champions for the hospital and promote carer engagement in service users’ treatment as appropriate
- To support and encourage visits by making sure visits are supportive and beneficial to service users
- To ensure child visits are in the best interest of the child and properly risk assessed prior to a visit being approved
- To identify, assess and contribute to the management of risk
- To act as a lead in the multi-agency contact relating to matters of Safeguarding At Risk Adults, Children and Young People, acting as Appropriate Adult for police interviews, and supporting MAPPA/MARAC needs

What you'll bring to the role
Essential for the role – Social Worker
Applicants must hold a Degree in Social Work and be currently registered with Social Work England, with completion of the ASYE or equivalent post-qualifying experience. They should have experience managing a caseload independently and undertaking assessments, risk management, safeguarding enquiries, and care planning, as well as working with multi-agency partners. Strong knowledge of relevant legislation and policy is required, along with the ability to manage complex cases under appropriate supervision. Candidates must also demonstrate strong assessment, analytical, and decision-making skills, alongside the ability to produce professional reports and maintain accurate records.
