About Elderwood at Burlington

Elderwood at Burlington is a 150-bed skilled nursing and subacute rehabilitation facility located in beautiful Burlington, Vermont. We provide comprehensive skilled nursing services, long-term care, and rehabilitation programs including orthopedic rehabilitation, speech therapy, and physical therapy. Specialty services include palliative care, respite care, and wound care, allowing us to meet a wide range of resident needs.

Social Worker – Position Overview

As a Social Worker, you will play a vital role in supporting residents and their families by identifying psychosocial, emotional, and mental health needs and connecting them with appropriate resources and services. You will help foster an environment that promotes dignity, independence, individuality, and overall well-being. Working as part of the interdisciplinary care team, you will assist residents through admissions, care planning, transitions of care, and ongoing support throughout their stay.

Key Responsibilities

  • Welcome and assist new residents during the admission process
  • Assess residents' psychosocial, emotional, and support needs
  • Participate in the development and review of individualized care plans
  • Complete required admission and discharge documentation, including SCREEN forms when applicable
  • Assist with discharge planning and transitions to other levels of care
  • Document resident information according to facility policies and regulatory requirements
  • Provide counseling and support to residents and family members
  • Coordinate with interdisciplinary team members to address resident needs
  • Advocate for resident rights, dignity, and quality of life
  • Participate in departmental meetings and in-service training programs

Qualifications

  • Minimum of a bachelor’s degree in social work or a bachelor’s degree in a human rehabilitation counseling, and psychology from an accredited institution; and services/ field including, but not limited to, sociology, gerontology, special education.
  • One year of supervised social work experience in a health care setting working directly with individuals
  • Experience in casework required.
  • Valid Drivers License and clean driving record required.
  • Work experience in the admission or discharge area of social work very desirable.
  • Desire and ability to work with the elderly and to cooperate with other staff members necessary.
  • This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
