Job Title: Health Center Social Worker

Schedule: Full-Time | Monday – Friday
Pay: $36-38/hr., depending on experience

About the Role

The Health Center Social Worker serves as the licensed social worker for our Health Center and plays a critical role in coordinating all resident discharges. This position partners closely with residents, families, clinical teams, and external agencies to ensure smooth transitions, emotional support, and access to vital resources.

You’ll be a trusted advocate, a collaborative teammate, and a calm presence during some of life’s most challenging moments.

What You’ll Do

Resident Advocacy & Social Services

  • Assess resident psychosocial, emotional, and medically related social needs.
  • Provide direct social services or connect residents with appropriate community resources.
  • Assist residents with financial assistance applications and outside agency referrals.
  • Maintain accurate, confidential social service documentation and progress notes.
  • Educate residents and families on resident rights.

Care Planning & Assessments

  • Complete initial social service assessments for all admissions.
  • Maintain MDS social service sections and conduct quarterly assessments for LTC residents.
  • Participate in and coordinate interdisciplinary care plan meetings.

Discharge Planning & Transitions

  • Serve as the facility’s Discharge Planning Coordinator.
  • Coordinate admissions, transfers, and discharges with internal teams and external partners.
  • Communicate discharge and transfer reports.
  • Track Health Center days for independent living residents.

Admissions & Community Engagement

  • Partner with the Admissions Coordinator to provide tours and admission information.
  • Serve as backup for admissions processes, including interviews, documentation, and financial data collection.
  • Collaborate with Finance, Marketing, and other interdisciplinary teams.
  • Attend weekly Levels of Care meetings.
  • If applicable, assist with clinical liaison marketing efforts and discharge follow-ups.

Qualifications We’re Looking For

  • Bachelor’s degree in Social Work, Gerontology, or a related field.
  • Current Social Worker license in the state of CT.
  • Minimum of 2 years’ experience in senior services or a related field.
  • Strong communication, documentation, and interpersonal skills.
  • Professional presence and the ability to navigate emotionally complex situations with grace.
