Job Description

As a member of the Interdisciplinary Team, provides psycho-social support to hospice patients.

JOB REQUIREMENTS

Minimum Education

Degrees allowed:

  • Has a baccalaureate degree in social work from an institution accredited by the Council on Social Work Education; or
  • A baccalaureate degree in psychology, sociology, or other field related to social work and is supervised by an MSW; and

-Has 1 year of social work experience in a healthcare setting; or

-Has a baccalaureate degree from a school of social work accredited by the Council on Social Work

  • Education, is employed by the hospice before December 2, 2008, and is not required to be supervised by an MSW.
  • Social work supervision: The Conditions of Participation require that, after the December 2, 2008 implementation deadline, the social workers that have BSWs and other degrees will need to have MSW supervision.
  • BLS certification.
  • Valid Drivers license.

Required Skills

  • Two years experience as a social worker in hospital, long term care or hospice setting.

ESSENTIAL FUNCTIONS

Essential Functions

  • Performs/documents psychosocial assessment.
  • Attends the Interdisciplinary Team and/or directly facilitates patient-family meetings for goal setting and care planning.
  • Provides liaison function between the Interdisciplinary Team, discharge planning, and social service departments.
  • Provides liaison function between the Interdisciplinary Team and hospice, home care agencies, long term care facilities, and other community services as needed.
  • Leads the Bereavement Support Group and facilitates referrals for advanced grief counseling if necessary.
