Cobble Hill is currently seeking a compassionate and dedicated Social Worker to join our interdisciplinary team. This is a confidential search for a professional who is passionate about advocating for residents, supporting families, and helping ensure the highest quality of life for those in our care.

The Social Worker is responsible for supporting residents' psychosocial well-being, assisting with care planning, coordinating discharge planning, and serving as a liaison between residents, families, and the care team. The ideal candidate is empathetic, organized, and experienced in skilled nursing or long-term care settings.

While LMSW licensure is not required, candidates who hold an LMSW are strongly preferred.

Responsibilities

• Conduct comprehensive psychosocial assessments upon admission and as needed
• Assist in developing, implementing, and documenting individualized care plans
• Provide emotional support and counseling to residents and families
• Coordinate discharge planning and community resource referrals
• Participate in care plan meetings and family conferences
• Assist residents with advance directives and end-of-life planning
• Advocate for resident rights and well-being
• Collaborate with nursing, therapy, and administrative teams to ensure coordinated care
• Maintain accurate and timely documentation in accordance with DOH regulations

Qualifications

• Bachelor's or Master's Degree in Social Work
• LMSW license in New York State preferred (not required)
• Experience in a skilled nursing facility or long-term care setting preferred
• Strong understanding of Medicaid, Medicare, and discharge planning processes
• Excellent communication and interpersonal skills
• Ability to handle sensitive situations with professionalism and compassion
