To provide quality social work services congruent with the Code of Ethics and Standards of Practice of the Ontario College of Social Workers and Social Service Workers. Reporting to the Manager of the Schizophrenia Outpatient Clinic, the registered Social Worker will utilize a full scope providing consultation to interprofessional team members and clinical case management to individuals diagnosed with serious and persistent mental illness.
QUALIFICATIONS:
•Clinical MSW and/or BSW from an accredited university. MSW is preferred
•Current registration with the Ontario College of Social Workers and Social Service Workers
•Membership with the Ontario Association of Social Workers an asset
•Minimum of two years demonstrated experience completing clinical case management in an outpatient mental health setting.
•Demonstrated experience in clinical case management in outpatient agency required
•Demonstrated expertise in psychosocial assessment of individuals and families
•Demonstrated knowledge in Mental Status Examination and suicide risk assessment
•Demonstrated commitment to ongoing professional development, including additional certification in counselling modalities specifically Cognitive Behavioural Therapy; Dialectical Behavioural Therapy and Concurrent Disorders and/or smoking cessation
•Comprehensive knowledge in mental health diagnoses and related treatments including familiarity with psychotropic medications and recovery oriented processes of care required
•Comprehensive knowledge with ability to integrate evidence based methods and relevant best practices for this target population required
•Comprehensive knowledge of regional community resources , including both acute and specialized services required
•Superior skills in advocacy to negotiate systemic challenges and remove patient access systemic barriers required
•Ability to work and problem solve independently required
•Strong interpersonal skills to function as a productive member of an interprofessional team with understanding of relevant roles required
•Working knowledge and experience with relevant legislation, including the Mental Health Act, Health Care Consent Act and Substitute Decisions Act essential
•Ability to organize and prioritize multiple and competing demands
•Accurate and timely documentation, meeting professional and organization standards
•Demonstrated commitment to mentoring social work students
•Interest and demonstrated abilities in program planning and development and research preferred
•Good work performance and attendance record are essential
•Valid drivers licence and access to a vehicle with appropriate insurance
