Salary starting at $62,000 to $77,200 Annual Salary (Salary is based on years of experience).

Schedule: Full Time- (8am-4pm) - Monday to Friday.

Westminster Place is seeking a dedicated Full-Time LSW Social Worker for the Healthcare Department. Essential Duties:

  • Deliver short and long-term supportive counseling to help residents manage transitions, discharge planning, loss, and adjustments to physical, cognitive, and emotional changes.
  • Serve as an advocate and liaison for residents, addressing health and psychosocial needs with their families.
  • Foster effective communication among residents, families, and staff.
  • Facilitate interdisciplinary service plan meetings and manage crises, assisting residents and families during incidents such as wandering, pre-hospice needs, and behavioral issues.
  • Assist residents in accessing services to maximize their independence and support their physical and psychosocial needs.
  • Develop and implement educational and supportive programs for residents and their families.
  • Inform residents of their rights and advocate to ensure these rights are upheld.
  • Conduct psychosocial assessments and document findings in compliance with state and federal guidelines.
  • Utilize software and computers to enhance efficiency in service delivery and documentation.
  • Demonstrate proficiency in electronic medical records (EMR) or achieve proficiency within the probationary period.
  • Participate in interdisciplinary committees addressing resident psychosocial needs and institutional concerns.
  • Provide in-service training to staff on the psychosocial needs of residents and the older adult population.
  • Adhere to Presbyterian Homes Corporate Compliance policies, ensuring confidentiality, security of information, and resident well-being.

Education: Master’s Degree in Social Work, LCSW Preferred.

Experience: A minimum of two years post graduate social work.

Qualifications:

  • Licensed Social Worker in Illinois.
  • Strong counseling and communication skills.
  • Active participant in interdisciplinary healthcare teams.
  • Proactive, team-oriented, and flexible, capable of multitasking and problem-solving.
  • Experienced in group facilitation and maintaining confidentiality.
  • Passionate about working with older adults.
  • Experience with Point Click Care EMR system is a plus.
