We currently offer an opportunity for an outpatient, Full Time, day shift, Social Worker to support to support oncology patients in our outpatient cancer center in Cleveland.

A Brief Overview
Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves as a resource within the social work department and the hospital.

Preferred Qualifications:
Post-Master’s experience is strongly preferred. Previous work in oncology or chronic diseases is also preferred. This is an outpatient role within the Seidman Cancer Center in Cleveland. This position supports half of our Bone Marrow Transplant, Malignant Hematology, and CAR-T clinics. The Social Worker addresses social determinants of health, patient needs, completes assessments required for insurance coverage, provides linkages to resources like Hope Lodge, focuses on the emotional needs and family dynamics, arranges for transportation, etc. They also communicate daily with the providers to ensure patients are moving appropriately along the transplant path.The schedule is Monday-Friday, weekdays.
