The Social Worker 2 role within the University Hospitals Lake West Medical Center offers a unique chance to become part of a dynamic, collaborative interdisciplinary team. This position adds value to patient care and significantly assists individuals with various issues.
We currently offer an opportunity for a PRN Social Worker to support patients in Willoughby.
A Brief Overview
Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves as a resource within the social work department and the hospital.

Position Details
Previous work in a hospital setting is strongly preferred. This PRN position will travel between Lake West and TriPoint Medical Centers. The Social Worker will be required to work in any unit of the hospital as needed, to perform psychosocial assessments, provide support to patients and families, and assist with discharge planning. It is needed to cover weekdays and weekends.
