Responsible for providing psychosocial-emotional support and services to patients and families. Assists patient and family in understanding, coping and adapting to the personal, emotional and bio-psychosocial issues associated to the patient's illness. Provides comprehensive assessments and referrals, crisis intervention and care management. Collaborates with multi-disciplinary team and other internal and external personnel to promote an awareness of environmental and psychosocial needs and to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community. May provide functional direction to unlicensed social work staff and social work interns.
