Social Worker x 2 | 10565 | Permanent contract | 37 hours per week | £39,862 to £41,771 per annum | (Grade J) | Holt or North Walsham, Norfolk

We offer a £2000 welcome payment for qualified and experienced Social Workers/Occupational Therapists (grade J and above), Practice Consultants and Team Managers new to Norfolk County Council. Amount to be repaid in full should you leave within two years of employment. 

For experienced Social Workers new to Norfolk County Council, we promise to try and match or better your existing salary, providing it is in the spectrum of the grade advertised.

An exciting opportunity has arisen for qualified Social Workers to join our dedicated team supporting adults during inpatient stays. This is a rewarding role where you will work at the heart of a multi-agency team to facilitate safe and timely discharges, ensuring individuals receive the right support at the right time.

About the Role
We have 2 full time roles available: one at Kelling Hospital, one at North Walsham Hospital. In this role, you will collaborate closely with health and social care professionals to deliver high-quality, person-centred care. Our service operates under a Home First ethos, prioritising independence and recovery through strong partnership working.


As part of a small, dynamic team, you will:
• Undertake person-centred assessments with patients and their families.
• Develop and implement care plans that promote independence and wellbeing.
• Work collaboratively with multi-disciplinary teams to ensure smooth discharge planning.

To build meaningful relationships with the people we support, this role requires regular in-person work in both the office and community

About You
We are looking for a Social Worker who is passionate about making a difference and committed to delivering excellent care. You will have:
• A sound understanding of the Care Act 2014 and its application.
• Experience in completing Mental Capacity Assessments and acting in a person’s best interests.
• Knowledge and experience of Continuing Healthcare (CHC) reviews.
• Strong communication and organisational skills, with the ability to work effectively in a fast-paced environment.
