Social Work Case Manager (LCSW, LICSW or LMHC)

Baystate Health/Health New England Services, a nationally recognized leader in healthcare quality and safety and is looking for a Social Work Case Manager to join our team.

  • 40-hours a week

  • Monday-Friday

  • No weekends, no holidays

LOCATION:

  • Remote Work Option - Must live in MA or CT for commutable requirements

QUALIFICATIONS:

  • Minimum Requirements:

  • Master’s degree in Social Work, Psychology, Education, PublicHealthor a related field with a minimum of three years of experience working with individuals with complex medical,behavioraland psychosocial needs.

  • Current state licensure as a social worker or related field

  • Experience in patient education working with individual patients

  • Knowledge of chronic health conditions preferred

  • Must be self -starter willing to proactively seek out and learn about etiology,careand treatment for many healthconditions

  • Excellent verbal and written communication skills in both English and Spanish preferred

  • Computer literacy and ability to navigate through computer systems

  • Skilled at deliver presentations

  • Sound assessment and decision-making skills, and highly ethical

  • Excellent critical thinking and organizationalskills,withattentiontodetail

  • Excellent time management skills and ability to manage multiple priorities

  • Strong teamwork skills; ability to work collaboratively and consultatively with team members

  • Ability to work independently and manage an assigned caseload

The Social Work Case Manager collaborateswiththe BehavioralHealthManager,NurseCaseManager,CareCoordinators,BehavioralHealthCareManagers, and medical health clinicians to deliver social work case management services to members in compliancewithcurrentaccreditationrequirementsandqualitystandards,andwithinthescopeofthemembers’benefitpackageandperHNEpolicies.Arrange,coordinateandadvocateforservicestomeetthepsychosocialneedsofthememberandifindicated,thefamilyandcaregiversofthemember.SupportstheHNEmissionbydevelopingpartnershipswithandprovidingpersonalandaccountableservicetomembersandproviders.ImpactpositivehealthoutcomesformembersandpositivefinancialoutcomesforHNE.Keepabreastofprofessionalstandards,newtechnologiesandproactivelyseekoutnewlearning.Availabletocoverweekends,eveninghoursandHolidaysasneededtoensureorganizationalcompliancewithrequirementsfromNCQA,DOIoremployergroupcontracts.AlsowillprovideminimumUMsupport as needed.

Essential Functions:

MemberSupportandEducation

  • Outreachesto assess member’s social, emotional, environmental, financial and support needs which areimpactinghealth outcomes

  • Providesor arrangesfor resources necessary to meet members’ psychosocial needs

  • Effectivelyutilizesmethods and models of social work intervention to engage members in care management

  • Providesongoing health coaching and support, self-management education, condition-specific education, care coordination, and resources and materials

  • Assistsmemberswhen necessary, in placing calls, completing applications, and advocating for available supports/services

  • Assistsmembers and providers with transition of care, referral management, care coordination, and benefitmanagement

  • Linksmembers toappropriate communityresources for health and health-related services consistent with regulatory,contractual,and corporate requirements

  • Researchesand sharescommunity resources with members and the case management team

  • Promotesandencouragesmembercollaborationwiththeprimarycareprovider,otherhealthcareprovidersandsocialserviceagencies

  • Facilitatesinterdisciplinaryconsultationonmembers’behalfthroughparticipationinrounds,teammeetingsandclinicalreviews

  • Conductsface-to-facevisitswithselectedmembersasappropriate

  • Completesdocumentationinthemedicalmanagementinformationsysteminatimelymannerandinkeepingwithinternalpolicyandaccreditation standards

  • Conductsassessmentsviaphoneanddevelopaserviceplanwhichoutlinescleargoals,timeframesandresponsibilities

  • Adherestodepartmentallyestablishedproductivitystandards

  • MaintainsHIPAAstandardsandconfidentialityofprotectedhealthinformation

  • Adherestodepartmental/organizationalpoliciesandprocedures

  • Understandstheconcepts,features,benefits,disclosuresandprovisionsofgovernmenthealthinsuranceprograms

InternalCustomerSupportandEducation

  • WorkscollaborativelyandcohesivelywithallmembersoftheCSITeaminordertocoordinateacontinuumofcareformembersconsistentwiththemembers’healthcaregoalsandneeds

  • FacilitatesmembercommunicationwithinternalpartnerssuchMemberServices

  • Provides support for BH UM responsibilities as needed

Note: The compensation range(s) in the table below represent the base salaries for all positions at a given grade across the health system. Typically, a new hire can expect a starting salary somewhere in the lower part of the range. Actual salaries may vary by position and will be determined based on the candidate's relevant experience. No employee will be paid below the minimum of the range. Pay ranges are listed as hourly for non-exempt employees and based on assumed full time commitment for exempt employees.

$66,081.00 - $75,982.00 - $89,876.00
