Job Summary

Performs 100% intake and psychosocial assessment on all patients and their families. Coordinates services and offers individual, family or group counseling and discharge planning. Acts as primary contact with third party payers and their representatives. Facilitates case management function with coordinators for third party issues.

Essential Functions

  • Interviews all patients and family members, assesses their individual situations with regard to psycho-social situation and emotional adjustment to illness, formulates a clinical impression, and projects a discharge plan.
  • Conducts educational/support groups for the population served
  • Facilitates case management services. Acts as primary hospital liaison with insurance case managers
  • Orients patient/family to Burke programs, policies, personnel, team members and rehabilitation philosophy and practice.
  • Coordinates team efforts to assure that the patient’s discharge planning proceeds in an orderly, purposeful and goal directed manner.
  • Communicates effectively and works collaboratively with medical, nursing, clinical services, and insurance case managers to provide comprehensive medical care. Participates in medical rounds and team conferences.
  • Defines and interprets social/emotional material to the physician and other members of the health team. Provides consultation on emotional, social and financial aspects of care to hospital staff.
  • Promotes participation of patient/family in development of plans and goals, and in discussion of patient progress and perception.
  • Coordinates and is responsible for discharge planning. Locates resources, secures insurance approval, and refers to community agencies.
  • Communicates information regarding pt’s progress to appropriate persons (i.e.designated discharge planners, insurance case managers)
  • Utilizes supervision appropriately. Attends and participates in educational programs for professional development.
  • Identifies opportunities for continuous practice based performance.
  • Oversees coordinators and/or students.
  • Develops and participates in research activities that emanate from worker’s assignment.
  • Provides Social Work services to other staff and serves as a resource as described in the Employee Assistance Program.
  • Maintains confidentiality and complies with professional ethics according to professional and hospital standards.
  • Documents in the medical record according to division/hospital policy.
  • Develops and maintains positive staff relationships.
  • Understands and follows division and hospital policies. Attends staff meetings and participates in program problem solving activities.
  • Covers for their specific covering worker and other social workers in the hospital
  • Assists with supervision of SWCMs, coordinators, students.
  • Develops research activities that emanate from worker’s assignment as well as engage in other research projects of the division.
  • Performs other duties as directed by the Director of Care Management.
  • Demonstrates the ability to translate knowledge of physical psycho-social and cognitive perceptual changes related to age into assessments, treatment plans and treatment modalities for pediatric, adolescent, young adult, adult and geriatric patients.
  • Practices universal precautions.
  • Personally adheres to Service Excellence standards by demonstrating positive communication, prompt & respectful service and maintaining an appropriate work environment when working with patients, staff, co-workers, visitors and others.
  • Other duties as assigned.

Qualifications

  • Maintains caseload of 20-25 patients and their families.
  • Member of team composed of 8-10 people and performs a coordinating function.
  • Has key relationship with M.D., nurse and core team. Relates to all disciplines on a daily basis.
  • Regular interaction with finance areas, utilization review and other support services.
  • Has contact with referral sources and is responsible for discharge planning and community linkage services.
  • Has primary responsibility for communicating with external case managers
  • Ability to interact with varied backgrounds of both professionals as well as patients and families.
  • Must be skilled in communication and be able to deal with concepts in language of the listener.
  • Ability to mediate and make assessments.
  • Coordinate in objective fashion.
  • Embodies the essence and intent of Service Excellence.
  • Licensed Master Social Worker - New York State Board of Social Work Required
