Reporting To: Heather Renfro

Compensation: $19.00 - $24.00 / hour

Schedule: Full Time | 40 hours per week | Monday - Friday | 8:00 am - 4:30 pm

What You'll Do In This Role:
Make a meaningful difference every day as a Social Work Assistant at Huron Valley PACE! In this rewarding role, you'll help older adults access and maintain vital benefits by assisting with Medicaid redeterminations, Food Assistance Program (FAP) applications, and Patient Pay processes. Working alongside the Social Work team and partnering closely with the Michigan Department of Health and Human Services (DHHS), you'll play a key role in connecting participants to the resources they need while supporting seamless, person-centered care.

Key Responsibilities

  • Assist participants and families with Medicaid redeterminations, Food Assistance Program (FAP) applications, Patient Pay, and other benefit-related forms while working closely with MDHHS.
  • Coordinate participant referrals, care transitions, and access to community resources to support seamless, person-centered care.
  • Monitor Medicaid/FAP eligibility, temporary placements, and case updates through CHAMPs while maintaining compliance with PACE and MDHHS requirements.
  • Guide participants and families through applications, program requirements, Advance Directives, guardianship processes, and required placement documentation.
  • Communicate participant status changes with the Social Work team and Interdisciplinary Team (IDT) to ensure continuity of care.
  • Build positive relationships with participants, families, and community partners to connect individuals with the resources they need.
  • Maintain accurate, timely documentation in the Electronic Medical Record (EMR) and report required updates to MDHHS.
  • Collaborate with Social Workers, the IDT, Medicaid Specialists, and other team members to support participant care and ongoing caseload management.
  • Utilize Microsoft Office, EMR systems, and strong organizational skills while contributing to special projects and other department initiatives as needed.

Skills, Knowledge & Expertise

  • Bachelor degree in a human service related field
  • One year experience providing resource coordination
  • Experience working with the elderly
  • Must have reliable transportation and a valid driver's license
  • High School Diploma or GED required
