Responsible for managing all aspects of the provision of patient social services including managing the members of the social work team. Establishes, implements, and evaluates goals and objectives for social services to meet and promote the standards of quality and contribute to the organizations mission and philosophy.
DUTIES & RESPONSIBILITIES
1. Collaborates with hospitals and community agencies to facilitate continuity of care as needed
Assesses social, psychosocial and emotional factors related to patients’ illness to determine ability to cope with death and the dying process.
Assesses grief needs, suicide risk and interventions.
Assists with discharge planning and has a knowledge of community resources available for patient needs.
Facilitates difficult conversations around goals of care, Advanced care directives and family dynamics between patients and loved ones.
Represents the hospice team in the community when meeting with families and patients to discuss services.

lll. JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
Master’s degree in social work. LCSW/MSW Required At least 1 year Hospice Collaborates with hospitals and community agencies to facilitate continuity of care as needed. Knowledge of corporate business management, governmental regulations and standards. Demonstrates good communications and public relations skills. Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.
