The Social Services Coordinator at Burnett Extended Care Center is responsible for providing psychosocial-emotional support and services to residents and families. Assists residents and family members in understanding, coping and adapting to the personal, emotional and psychosocial issues associated with the resident's illness, disability, or recovery. Provides comprehensive assessments, referrals and care management. Collaborates with the interdisciplinary team and other internal and external personnel to promote an awareness of environmental and psychosocial needs and to develop and implement treatment plans that support resident-centered plans of care for residents.
Education• High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required Experience• 2 years of social services experience preferred• Long-term care social services experience preferred• Experience with PointClickCare or EHR systems (preferably in a Skilled Nursing facility/setting) preferred
