Your Role as a Social Services Coordinator:

• Provide social services support to residents, addressing psychosocial, emotional, and environmental needCommunicate with residents and families to resolve concerns, provide guidance, and ensure needs are met
• Complete and maintain comprehensive assessments, including social histories, care plans, and progress notes
• Advocate for residents’ rights, ensuring dignity, privacy, and respect at all times
• Collaborate with interdisciplinary teams, including staff, consultants, and healthcare professionals
• Maintain strict confidentiality of resident information in accordance with HIPAA regulations
• Support a calm, caring, and respectful environment for residents and staff
• Participate in care planning and contribute to individualized resident care strategies
• Attend required trainings, in-services, and meetings

What We're Looking For:

• Bachelor’s degree in Social Work or related field from an accredited college or university
• Current Social Worker license in the State of Ohio
• Minimum of 2 years of experience in a healthcare setting preferred
• Strong problem-solving, organizational, and multitasking skills
• Excellent communication and customer service abilities
• Ability to maintain accurate and timely documentation
• Ability to work both independently and collaboratively as part of a team
• Compassionate, calm, and professional demeanor in emotionally demanding situations
• Ability to read, write, and speak English
