Job Description

Reports to: Director of Long Term Care

FLSA Classification: Full-Time, Non-exempt, Hourly $24.00-$28.00

Essential Functions:

  • Assist the Interdisciplinary Team with identifying, planning, implementing, and evaluating for identified emotional and social needs of each resident to enhance resident psychosocial health and wellbeing.
  • Begin the pre-admission process by assessing the prospective resident and determining appropriateness of placement from a psychosocial perspective.
  • Serve as the admission coordinator, facilitating admissions process for prospective residents and their families. Explain and assist with appropriate and necessary paperwork.
  • Act as a liaison between the Walbridge Wing and community resources as needed and appropriate to meet the needs of residents.
  • Assist residents and families in coping with the medical and psychosocial aspects of the resident's illness, disability, and/ or stay in the facility.
  • Assist in the referral process for mental health support services when needed.
  • Coordinate non-medical transfers. Assist residents in adjusting to such transfers.
  • Participate in resident assessment and care planning.
  • Record information on social history in the health record; review record annually.
  • Record progress notes on resident's health record at least quarterly for the first six months.
  • Assist in developing policies and procedures pertaining to social services within the facility.
  • Provide orientation to new residents nd their families.
  • Act as resident advocate by providing assistance to residents and families raising concerns about resident care, and assist in investigation of such concerns.
  • Assist patients in planning for discharge.
  • Document appropriate information within patient charts.
  • Coordinate discharge planning with physicians, nurses, families, community agents, and families.
  • Other duties as assigned

Education and Experience:

  • Bachelor's Degree in Social Work or related field required.
  • Master's Degree in Social Work or related field preferred.
  • IF not a Licensed Social Worker, must complete required Social Services Designee for Long Term Care Training.
  • Previous experience in a long term care setting preferred.
  • Current BLS required within four (4) months of transfer or hire.

  • May be required to wear N95s or PAPRs up to 80% of the shift.

About Pioneers Medical Center

Pioneers' history began in 1947 when Freeman Fairfield, a previous resident of this community, approached the County Commissioners about establishing a hospital. Over the next several years, Mr. Fairfield worked with the Commissioners and a group of local citizens to build a hospital for the Meeker community. Opening its doors in 1950, Pioneers began offering acute health care and surgical services to the community, licensed through the State of Colorado Department of Health and Human Services. We are Medicare and Medicaid-certified, accept private insurance, and, of course, self-pay. Financial arrangements may be set up through the Business Office.

In 1964, the Walbridge Memorial Convalescent Wing opened its doors for long-term care, made possible through the generosity of the wills of Carl and Adelaide Walbridge. Attached to the north side of the hospital, the Wing, with its 33 beds, brings skilled and intermediate nursing to residents who are unable to care for themselves. Through the Wing's rehabilitation services, exercise, and activity groups, each resident's care is individualized to meet his or her physical, emotional, nutritional, and spiritual needs. The Wing is licensed by the State of Colorado Department of Health and Human Services, and accepts Medicare, Medicaid, and private-pay residents.

First-line health care services are delivered through Meeker Family Health Center. The MFHC originated in 1990 as a rural health clinic, and its physicians, nurse practitioners, and staff continue to strive to meet our community's and visitors' healthcare needs.
