Integrated Sleep Care is a Sleep Telemedicine and remote physiological monitoring (RPM) company focused on optimizing patient experience with sleep medicine diagnostics and therapy. The company incorporates coaching and RPM software support to improve adherence with CPAP treatment for obstructive sleep apnea.

POSITION SUMMARY
The Sleep Care Coach is a critical member of the operations team. This position is 100% remote based and is responsible for providing coaching, education and support to patients. This position will require interactions with patients, caregivers, providers, and provider teams.
PRIMARY DUTIES AND RESPONSIBILITIES include the following:
· Provide day-to-day review and follow up on individual patient activity as presented in the Sanusom Portal with “Get Next Patient.” Please note this is “Report Only;” no interpretation or clinical management of data
· Initiate outbound calls to patients and/or caregivers who need attention
· Answer inbound calls from patients and/or caregivers who have questions regarding their care
· Provide ongoing support, encouragement, and accountability to ensure patients stay motivated and committed to their sleep improvement goals, creating a high CPAP adherence rate
· Interact with DMEs and provider offices via email/phone as appropriate on behalf of the patient for items such as general equipment troubleshooting, mask refits, pressure changes, etc.
· Document all actions and interactions with the patient in the Electronic Sanusom Portal and the Electronic Medical Record
· Collaborate, train, and support new staff (peers)
· React to change productively and handle all other duties/essential tasks as assigned
· Be available and online as the schedule is designed to support patient care
· Attend individual and team meetings; ensure collaboration and teamwork
· Communicate effectively (both orally and written)
· Report any issues or concerns appropriately to leadership using appropriate technology platforms
· Assist patients in making new appointments and/or adjusting existing appointments (as appropriate)

EDUCATION AND/OR EXPERIENCE
· Previous experience in counseling, coaching, or healthcare, particularly in sleep-related fields is preferred

· Strong interpersonal skills; ability to communicate effectively with a diverse range of individuals to include patients and/or caregivers, providers, DMEs, etc.; ability to build rapport with patients and effectively convey information about sleep health

· Computer literate with working knowledge of electronic medical records, Microsoft Word and Excel, Call systems, and electronic portals

WORK ENVIRONMENT
· This is a fully remote-based position that involves everyday risks or discomforts that require normal safety precautions.
· Must have a suitable space to set up a home office to mitigate distractions that allows he/she to maintain a focused and productive work environment.
· Must be able to effectively manage and minimize distractions in the remote home office setting.
· Home office equipment to be provided includes computer, keyboard, mouse, and monitors.
· Must have reliable internet service to avoid interrupted delivery of service.
