We are currently seeking a full time day Skill Builder I for our Connxxions program located in Delaware County, IN. This position will provide skills and development training to assist patients and/or families in developing skills directed towards achieving developmental milestones and reducing psychosocial barriers. A Skill Builder provides case management services, working in conjunction with the clinical team for coordination of care. A Skill Builder works with the patient on a day to day basis using professional judgment and discretion to implement the team determined treatment plan.
