Job Description

The CHOICES and Aged Care Portfolio provides a range of person centred services to support people living with disability, older people, and individuals experiencing mental ill health. Our services are focused on promoting independence, wellbeing and meaningful participation in the community. We work alongside each individual to understand their goals and strengths, supporting them through recovery focused approaches, skill development and opportunities for greater community inclusion.

The Role:

As the Seniors in Networked Communities (SINC) Administrator, you will work across the Canberra & Goulburn Archdiocese, including the ACT and Southern Highlands regions, to support the effective coordination and delivery of aged care services under the Commonwealth Home Support Program (CHSP).

CHSP provides entry-level support to people aged 65 years and over, or 50 years and over for Aboriginal and Torres Strait Islander people, to assist them to remain living independently and connected to their community.

You will work within a person centred, wellness and restorative approach, in accordance with the CHSP Manual and relevant Aged Care Quality Standards. You will play an important role in ensuring clients receive appropriate, timely and coordinated services that align with their individual needs and My Aged Care support plans.

Key responsibilities include, but are not limited to:

  • Managing referrals and maintaining the client waitlist.
  • Coordinating the onboarding of new clients to the program.
  • Liaising with the rostering team to support the timely delivery of services.
  • Undertaking rostering responsibilities as required.
  • Liaising with clients to understand their needs and ensure services are delivered in accordance with their My Aged Care support plans.
  • Monitoring changes in client needs and referring clients back to My Aged Care where their circumstances or support requirements change.
  • Maintaining accurate and timely client records and documentation.
  • Working collaboratively with internal teams and external stakeholders to support high-quality service delivery.
  • Ensuring services are delivered in accordance with CHSP requirements, organisational policies and procedures, and the Aged Care Quality Standards.
  • Participate in On Call as per the On Call Policy and Procedure.

About You

The ideal person we are looking for has:

  • A Certificate in Aged Care or similar, with relevant experience in aged care or community services.
  • Strong computer literacy, including proficiency in Microsoft Office and client management systems.
  • Excellent communication and interpersonal skills, with the ability to build positive relationships with clients, colleagues and stakeholders.
  • A high level of attention to detail, with the ability to learn new systems and processes quickly.
  • Strong organisational and time management skills, with the ability to manage competing priorities, multitask effectively and meet deadlines.
  • An understanding of Equal Employment Opportunity (EEO), Work Health and Safety (WHS), confidentiality and privacy requirements.

The Royal Children's Hospital (RCH) has been providing outstanding care for Victoria's children and their families for over 147 years.

We are the major specialist paediatric hospital in Victoria and our care extends to children from Tasmania, southern New South Wales and other states around Australia and overseas.

With a passionate, highly skilled and committed staff campus wide of over 5,000, we provide a full range of clinical services, tertiary care and health promotion and prevention programs for children and young people.

We are the designated state-wide major trauma centre for paediatrics in Victoria and a Nationally Funded Centre for cardiac and liver transplantation.
