Salary Range: $44,600.00 - $53,800.00 Salary/year

- - - Supports the Social Health Solutions department. This position will be responsible for community health work and serve as a Community Health Worker (CHW) to help individuals and their families navigate and access community services, other resources, and adopt healthy behaviors. CHW assists individuals in overcoming barriers to health, social services, education, and employment. CHW also provides social support, informal counseling, and advocates for individuals and community health needs.

JOB RESPONSIBILITIES:
- Build and maintain trust-based relationships with individuals and their caregivers, while offering culturally relevant support and motivation.
- Provides education regarding long term services including assisting individuals in gaining access to any necessary services regardless of funding source.
- Provide culturally appropriate health education and information including chronic disease management education and coaching Individuals to improve individuals’ health and well-being.
- Maintains proper documentation of work activities according to the program requirements..
- Implements Pathways in accordance with the Pathways Community HUB Institute® Model to achieve the desired outcomes.
- Provides services under CMS Guiding an Improved Dementia Experience program (“GUIDE
- Program”) and any other care coordination program that the Agency may implement for the benefit of its consumers.
- Recognize and report any signs of family violence, abuse, and neglect.
- Advocates for client and acts as a liaison between the client/family and community service agencies (i.e., schools, hospitals, support groups, etc.).
- Counsels’ clients individually, in family or other small groups regarding plans for meeting needs and aids client to mobilize inner capacities and environmental resources to improve social functioning.
- Conducts phone calls with individuals and/or their representatives in accordance with program specifications and within required time frames.
- Facilitates enrollment in Agency programs as well as external programs.
- Contacts and networks with individual’s guardians to obtain signatures as needed.
- Provide support and follow-up as needed with individuals by using motivational interviewing skills and goal setting.
- Assist individuals to set health goals and attend doctor appointments with individuals.
- Obtains and distributes community service information to individuals and/or their representatives.
- Determines client's eligibility for services such as financial assistance, insurance, and other programs in place to assist individuals and refers individuals to community resources and other agencies to meet identified needs.
- Attend meetings, trainings and other activities as requested.
- Participates in training to maintain and improve job skills.
- Assists with and completes special projects as assigned.
- Performs duties with respectful communications & behaviors.
- Experience with Pathways and/or Guide preferred.

QualificationsList required/preferred qualifications for this position.
- Experience working in a multi-cultural setting.
- Basic computer skills required.
- Good communication skills
- High School graduate
