Job Title: Shahbaz (CNA) - Wegman Family Cottages, Full-Time Day Shift

Location: Wegman Family Cottages

Hours Per Week: 40

Schedule: 7am - 3pm, Monday - Friday and every other weekend requirement



This role supports elder-directed living by honoring each elder’s preferences, routines, and daily rhythms. As an empowered and adaptable team, this role is dedicated to fostering a welcoming home where elders are honored with dignity, choice, and care built on relationships. In addition to providing compassionate clinical support and assistance with personal needs, the Shahbaz partners with elders to support the life they choose to live each day. Candidates must be certified Nursing Assistants (CNA) and able to work collaboratively in a self-managed, team-oriented environment that promotes exceptional elder satisfaction and upholds our Core Values and Brand Promise.

KEY RESPONSIBILITIES:

• Support all aspects of daily living in a way that honors elder choice—bathing, dressing, toileting/bowel regimens, personal hygiene, feeding, comfort care—following individualized care plans and the elder’s stated preferences.

• Accurately record ADLs in Care Tracker, give and receive shift reports, and actively participate in the Care Plan process as an empowered member of the team.

• Perform designated exercises, repositioning, and skin evaluations, and promptly report changes in condition to the care team.

• Provide emotional support to elders and families, respond promptly to requests, and guide families to appropriate professional staff, fostering a relationship-based home environment.

• Maintain knowledge and compliance with all Rochester Regional Health policies and all federal, state, and regulatory requirements.

• Prepare and serve meals in a family-style dining model, ensuring elders have meaningful choice in what and when they eat; assist with transporting residents to activities; and maintain a clean, organized home by completing light housekeeping tasks, including laundry, as directed by elders.

• Kitchen: Coordinate family-style dining and ensure the success of the overall dining experience. The role is responsible for cooking breakfast and washing dishes after all three meals while ensuring all safety and sanitation checklists are completed in full compliance with NYS Department of Health regulations.

MINIMUM QUALIFICATIONS:

• Completion of NYS approved certified nursing assistant program.

PREFERRED QUALIFICATION:

• High School Diploma or equivalent preferred.
• Valid NYS Drivers license preferred.
• Serve Safe Food Protection Manager Certification or Monroe County Health Department Level 1 Food Handler Certification for safe food handling preferred.
• Ability to read, write and speak English, ability to understand and carry out moderately complex oral and written instructions.
• Effective interpersonal skills. Ability to establish and maintain working relationships with other employees and the public.
• Ability to meet emotional and physical demands of elders.
• Basic knowledge of computer skills with ability to utilize email and Medical Record Documentation system.
• Requires proficient ability to exercise independent judgment under broad certified nursing care guidelines, as well as, food prep/planning/housekeeping/emergency areas and ability to prioritize critical tasks.
• Requires proficient ability to comply with Elder Rights, all regulatory requirements, and all Rochester Regional Health policies and procedures as stated now or as amended.

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
