Job Description

Catholic Social Services is currently recruiting for a contract, (37.5 hours/week), Settlement Counsellor - Health Navigator in Red Deer
This is a temporary position.

The Health Navigator plays a key role in supporting newcomers, particularly government-assisted refugees and newcomers with complex health needs. Working closely with the resettlement assistance and Case Management teams, the Health Navigator assists clients in accessing, understanding, and navigating the Canadian health care system while addressing barriers related to language, culture, trauma, and social determinants of health.

Job Duties:
• Client Support & Navigation
• Support clients to access primary care, specialists, mental health services, dental, vision, and community-based health supports
• Assist clients with health card applications, IFHP coverage, appointments, referrals, and follow-ups
• Provide health related orientation and education tailored to newcomer needs
• Accompany clients to medical appointments when required or arrange interpretation services
• Collaboration & Case Management
• Work collaboratively with RAP Counselors, Case Management counselors, and other settlement staff
• Participate in Case Management meetings
• Maintain accurate and timely client documentation and case notes
• Advocacy & Systems Navigation
• Advocate on behalf of clients with health care providers and systems
• Identify service gaps and barriers and support clients in overcoming them
• Build and maintain relationships with health care providers and community partners
What This Job Requires:
• Diploma or degree in Human Services/Social Sciences/Social Work or equivalent
• Experience working within RAP and/or Settlement Case Management programs
• Proficient with computer software including Microsoft Office and have the ability to learn new programs
• Experience supporting clients with health system navigation, referrals, and appointment coordination
• Strong understanding of the Canadian health care system, including primary care, specialists, mental health, and community-based services
• Strong organizational and time-management skills with the ability to manage multiple complex cases
• Fluent in Arabic Language is an asset
• Clear and Current Drivers Abstract
• A vehicle and valid class 5 driver's license – requirement to transport clients in your personal vehicle
• Proof of Vehicle Insurance showing $2 million liability coverage
• Proof of Vehicle Registration for insured vehicle
• Vulnerable Sector Check from local RCMP or city police detachment
• Child Intervention Record Check
