
The Food Farmacy Associate supports daily operations of a food-as-medicine program by packaging and delivering food to patients, coordinating deliveries and documentation in the EHR based on the task.
Minimum Qualifications
What You’ll Do
·Package and prepare food bags according to program guidelines
·Load, transport, and deliver food to patients safely and on schedule
·Verify deliveries and maintain accurate logs and documentation
·Communicate with patients and team members regarding deliveries, issues and delays
·Provide friendly, respectful service during patient interactions
·Support overall Food Farmacy operations, as needed

Work Environment & Demands
·Combination of time spent driving in the community, packing bags in the kitchen and assisting at the Food Farmacy pick-up location
·Regular lifting, carrying, and loading of bags (up to 25–50 lbs)
·Standing, walking, and repetitive tasks for extended periods
·Exposure to varying weather conditions during deliveries
·Fast-paced environment with set schedules and occasional route changes

·Deliveries are accurate, on time, and completed safely
·Food bags are packed correctly and meet quality standards
·Inventory is organized and discrepancies are minimal
·Positive, respectful interactions with patients and team members
