Job Description

The Service Delivery Coordinator plays a crucial role in ensuring the effective delivery of social care services in Ipswich, Queensland. This position is responsible for coordinating service operations, liaising with clients and service providers, and ensuring compliance with relevant regulations. The role is vital for maintaining high standards of care and support for individuals in the community, contributing to the overall mission of enhancing the quality of life for those we serve.

Responsibilities:

  • Coordinate the delivery of social care services to clients, ensuring their needs are met effectively.
  • Liaise with clients, families, and service providers to facilitate communication and service delivery.
  • Monitor service quality and compliance with relevant regulations and standards.
  • Assist in the development and implementation of service delivery plans.
  • Maintain accurate records and documentation related to service delivery and client interactions.
  • Provide support and guidance to team members to ensure high-quality service provision.
  • Participate in training and development activities to enhance service delivery skills.

Qualifications:

  • Associate degree or equivalent qualification in Social work or Psychology or related field.
  • Proven experience in a service delivery or coordination role, preferably within the community sector.
  • Strong organisational and time management skills.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively within a team and independently.
  • Familiarity with relevant legislation and best practices in NDIS & Child safety.
  • Proficient in using computer systems and software for record-keeping and reporting.
