Salary Range: $20.35 - $26.35 Hourly

Service Coordinator – Ghent, Norfolk
We are seeking a creative, detail‑oriented, and flexible Service Coordinator to join our Ghent team in Norfolk. This position requires travel to several supported individuals’ homes within the city.
Schedule
- Tuesday: 8:00 AM – 4:00 PM
- Wednesday: 9:00 AM – 5:00 PM
- Thursday: 8:00 AM – 4:00 PM
- Friday: 9:00 AM – 5:00 PM
- Saturday: 9:00 AM – 5:00 PM
- Base Pay: $20.35/hour
- Shift Differential: Additional $6.00/hour for Saturday shift

As a Service Coordinator, you will support individuals in living full, independent lives. This includes helping them set meaningful goals, manage medical needs, develop life skills, write support plans, and manage personal finances. This is a great role for someone who is organized, energetic, and passionate about person‑centered support.

Position Responsibilities
Daily Living & Skill Building
- Support individuals with cooking, cleaning, laundry, hygiene, meal planning, grocery shopping, transportation, banking, and medication management.
- Use your personal vehicle when providing services. We reimburse mileage.
- Teach skills based on each person’s Individualized Service Plan (ISP).
Community Integration
- Support participation in community activities and opportunities.
- Help individuals exercise their rights and assist in resolving concerns.
Behavioral & Medical Support
- Provide behavioral guidance and implement support plans as required.
- Administer first aid in emergencies and ensure timely access to medical care.
- Assist with scheduling and attending medical and dental appointments.
- Manage medications, prescriptions, and related documentation.
- Keep the team informed about any health‑related changes.
Documentation & Communication
- Maintain accurate and confidential documentation, including:
- - Medication notebooks
- - ISP notebooks
- - Permanent records
- Communicate relevant information to team members and act as a liaison with families, agencies, and community partners.
- Complete payroll, mileage, and other required forms.
Team Collaboration
- Attend weekly and monthly team meetings, quarterly agency meetings, retreats, and required trainings.
- Support team operations and provide coverage for the Team Leader when needed.
- Additional Responsibilities
- Assist individuals with errands as needed.
- Report maintenance issues promptly.
- Follow all agency policies and contribute to policy updates.

Qualifications

Qualifications
- Demonstrated ability to organize and prioritize work tasks effectively.
- Proficiency in Microsoft Excel, Outlook, and Word.
- Ability to work collaboratively as part of a team.
- Professional demeanor and ability to represent the organization appropriately.
- Ability to work independently, manage time well, and self‑direct daily tasks.
- Strong oral and written communication skills.
- Ability to participate in and contribute to a participative management structure.
- Ability to meet the physical requirements of the position, including:
- - Lifting up to 50 pounds several times per shift
- - Climbing stairs multiple times per shift
- - Sitting, standing, and walking as needed
- Knowledge of the field of disabilities.
- Skill in implementing behavioral support practices.
- Ability to complete all required training, including:
- - Medication Certification
- - Medicaid Waiver Certification
- - First Aid and CPR
- - Mandt
- - All required orientation trainings
- Strong judgment and decision‑making skills, especially during emergencies or crisis situations.
- Ability to maintain confidentiality of sensitive information.
- Skill in locating and accessing community resources.
- Valid driver’s license required.
- Satisfactory criminal history background check required.
