Service Coordinator
We are hiring a creative, detail-oriented, and flexible Service Coordinator for our Ghent location in Norfolk.
In this role, you will support individuals by helping them set goals, manage medical needs, write plans, and handle personal finances. This position is ideal for someone who is organized, energetic, and passionate about helping others live full, independent lives.
Schedule
Monday through Friday, 8:00 a.m. to 4:00 p.m.

Position Responsibilities
Daily Living and Skill Building
- Support individuals with daily living tasks such as cooking, cleaning, laundry, hygiene, meal planning, grocery shopping, banking, transportation, and medication management.
- Use your personal vehicle as needed while providing services.
- Teach skills based on each person's Individualized Service Plan.
Community Integration
- Help individuals participate in community activities.
- Support individuals in exercising their rights and resolving concerns.
Behavioral and Medical Support
- Provide behavioral guidance and implement support plans.
- Administer first aid in emergencies and ensure prompt access to medical care.
- Assist individuals with scheduling and attending medical and dental appointments.
- Manage medications, prescriptions, and related documentation.
- Keep team members informed about health-related updates.
Documentation and Communication
- Maintain accurate, confidential records, including:
- - Medication Notebooks
- - Individualized Service Plan Notebooks
- - Permanent Records
- Share relevant information with team members and serve as a liaison with families, agencies, and the community.
- Complete payroll, mileage, and other required forms.
Team Collaboration
- Attend weekly team meetings, monthly team meetings, quarterly agency meetings, retreats, and required trainings.
- Support team operations and fill in for the Team Leader when needed.
- Carry the team pager when requested.
Additional Responsibilities
- Assist with errands as needed.
- Report maintenance needs.
- Test fire alarm systems when required.
- Follow all agency policies and support future policy development.

- Demonstrated ability to organize, prioritize, and manage multiple work tasks effectively.
- Proficiency in Microsoft Excel, Outlook, and Word.
- Strong teamwork skills with the ability to collaborate effectively with colleagues.
- Professional demeanor and the ability to represent the organization appropriately.
- Ability to work independently, manage time well, and self‑direct daily tasks.
- Excellent oral and written communication skills.
- Ability to engage in and contribute to a participative management environment.
- Ability to meet the physical demands of the position, including lifting up to 50 pounds several times per shift, climbing stairs multiple times per shift, and sitting, standing, and walking as required.
- Knowledge of the field of disabilities and related best practices.
- Skill in implementing behavioral support strategies.
- Ability to complete all required training, including Medication Certification, Medicaid Waiver Certification, First Aid/CPR, Mandt, and all orientation training.
- Strong judgment and decision‑making skills, especially during emergencies or crisis situations.
- Ability to maintain strict confidentiality of sensitive information.
- Skill in identifying, accessing, and utilizing community resources.
- Valid Virginia driver’s license.
- Satisfactory criminal history and background check required.
