Job Description

About the Role

We’re seeking a full-time compassionate, community‑focused serious illness services Community Health Worker (CHW) to support individuals living with serious illness and their caregivers. As part of our Serious Illness Program’s special focus, you will provide education, navigation, and support that helps families navigate the journey of dementia more confidently and safely. The schedule for this position is 40 hours per week; weekends and holidays as needed.

What You’ll Do

  • Build trusted, long-term relationships with clients and caregivers
  • Conduct home visits to assess needs, safety risks, and caregiver strain
  • Provide dementia-specific education, communication strategies, and behavior‑management guidance
  • Support goals-of-care discussions and help families navigate care transitions
  • Connect clients to community resources, Medicaid/Medicare benefits, respite, and social supports
  • Advocate for families in medical and social service settings
  • Document interactions and collaborate with an interdisciplinary team

What We’re Looking For

  • Lived or professional experience supporting dementia, aging, or serious illness
  • CHW certification required
  • Strong communication, empathy, and cultural humility
  • Comfort working independently in clients’ homes
  • Valid driver’s license and ability to travel locally

Preferred Experience

  • Dementia training (Teepa Snow, Alzheimer’s Association, Dementia Friends)
  • Experience in palliative care, hospice, geriatrics, or behavioral health
  • Bilingual or bicultural candidates encouraged to apply
