About the Role
This role is responsible for delivering high-quality casework and support services to young people transitioning from state care to independent living. Working within a strengths-based and trauma-informed framework, you will provide practical support, advocacy, and mentoring to help young people achieve positive outcomes.
In addition to managing a caseload, you will support the Coordinator by mentoring and coaching new staff and students, leading reflective practice sessions, contributing to reporting requirements, and assisting with service administration. You may also act as the Coordinator's delegate during periods of leave and represent Uplyft at community and stakeholder events.

With a passionate, highly skilled and committed staff campus wide of over 5,000, we provide a full range of clinical services, tertiary care and health promotion and prevention programs for children and young people.
We are the designated state-wide major trauma centre for paediatrics in Victoria and a Nationally Funded Centre for cardiac and liver transplantation.

Our campus partners, the Murdoch Childrens Research Institute (MCRI) and The University of Melbourne Department of Paediatrics, along with the RCH Foundation, are on site with the hospital in Parkville.
