LEAP Program Description

Global Solutions Ventures (GSV) – a joint-venture partnership between ZemiTek, LLC and Dexis Consulting Group – is implementing the Long-term Exceptional Technical Assistance Project (LEAP Global), a mechanism originally used by USAID to battle against infectious diseases such as malaria, HIV/AIDS, tuberculosis, neglected tropical diseases, and pandemic influenza. As of July 1, 2025, LEAP Global transitioned to the U.S. Department of State (DOS). This initiative aims to address the complex human resource challenges faced by national infectious disease programs in the USA-supported countries.

The intervention of this project is to hire and embed technical expert advisors and subject matter experts within government entities. By directly embedding the technical expertise within the national programs, LEAP provides long-term, sustainable lifesaving support to empower host governments to lead and manage key aspects of their infectious disease portfolios and improve coordination between the U.S. Government and the host country.

About the Position

The Senior TB Financing Technical Advisor – Philippines will support the Philippine Department of Health (DOH) in developing and operationalizing a national Tuberculosis (TB) Investment Plan for 2027–2030, in line with the Philippine Strategic TB Elimination Plan, Phase 2 (PhilSTEP2), and in full alignment with the new Presidential Executive Order (EO) directives on life-saving humanitarian assistance (LHA) service delivery. The primary focus of the Advisor will be to provide high-level technical leadership on TB health financing, investment planning, and donor transition to ensure cost-effective, sustainable, and domestically financed TB service delivery.

Tuberculosis remains a major public health challenge in the Philippines, with one of the highest TB burdens globally and continued dependence on external assistance. Recent reductions in donor funding, including the closure of USAID TB support and a 25% decline in the Global Fund TB allocation for the 2027–2028 grant cycle, require a deliberate transition toward domestic financing. The TB Investment Plan will define a realistic, multi-source financing pathway to sustain and scale TB services through 2030 across national government, local governments, PhilHealth, development partners, private sector, and out-of-pocket spending.

Embedded within the DOH and working closely with the National TB Control Program (NTP) and relevant DOH offices, the Advisor will lead a consultative, multi-stakeholder process to:

  • Develop a long-term TB Investment Plan that aligns with national health priorities, PhilSTEP2, and global End TB targets.
  • Identify and prioritize practical strategies for financial sustainability and resource mobilization.
  • Support coordination and alignment of domestic and external financing (Global Fund, U.S. Government, other partners).

Responsibilities

The Senior TB Financing Technical Advisor will:

Initial Preparatory Work

  • Conduct a rapid desk review of existing TB strategic and operational documents, including:
    • Philippine Strategic TB Elimination Plan Phase 2 (PhilSTEP2, 2025–2030).
    • Existing TB investment cases, expenditure analyses, and fiscal space/financing assessments.
    • Prior Global Fund and U.S. Government TB funding requests, transition analyses, and sustainability plans
  • Map current TB funding flows and gaps across DOH, LGUs, PhilHealth, Global Fund, and other partners.
  • Develop a detailed work plan and stakeholder engagement plan for TB Investment Plan development, including timelines, key milestones, and decision points.
  • Agree with DOH on governance arrangements (Task Force/TWG) and validation mechanisms for the plan.

Stakeholder Consultations and Coordination

  • Design and facilitate structured consultations, key informant interviews, and focus group discussions with:
    • National government agencies (e.g., DOH – DPCB, UHC HSC, BLHSD, HPDPB, BIHC, PD, EB; DILG; DBM; CHDs and MOH BARMM).
    • Local Government Units (LGUs) and relevant local health boards.
    • PhilHealth units (MDG-TB, YAKAP, GAMOT and related benefit development/claims units).
    • Private sector health facilities and industry partners.
    • Development partners and donors (Global Fund, U.S. Department of State, other partners).
    • Civil society organizations, TB patient groups, and affected communities.
    • Public and private health facilities (hospitals, primary care facilities).
  • Facilitate validation meetings and technical workshops at key stages of plan development, including with:
    • DOH/NTP Task Force/Technical Working Group.
    • Global Fund GC8 preparation and GF transition planning teams.
    • S. Department of State counterparts under the TB Memorandum of Understanding (MOU).
    • Project development teams for potential ADB or other multilateral loans.
  • Ensure inclusive participation and consensus-building across stakeholders, with clear documentation of agreements and action points.

Development of the TB Investment Plan: Vision, Scenarios, and Strategies

  • Lead the development of a clear, shared long-term vision for TB financing through 2030, aligned with PhilSTEP2 impact and coverage targets and global End TB commitments.
  • Estimate and refine projected TB program resource needs for 2027–2030, using existing costing work and scenario analyses where available.
  • Analyze current and potential resource availability by source (DOH, LGUs, PhilHealth, Global Fund, other donors, private sector, households) and identify realistic trajectories for domestic financing growth and donor transition.
  • Define TB financing scenarios (e.g., minimal, moderate, and significant increases in DOH and PhilHealth funding) and their implications for service coverage, sustainability, and achievement of PhilSTEP2 targets.
  • Identify and prioritize key interventions and policy measures to expand domestic TB financing, including:
    • Increased and better executed DOH and LGU TB budgets.
    • Expansion and optimization of PhilHealth TB benefit packages, provider payment mechanisms, and claims management.
    • Integration of TB into UHC financing reforms and primary care benefits.
  • Recommend mechanisms to maximize health insurance reimbursements for TB services, including increased PhilHealth enrollment, improved provider billing and claims processing, and enhanced TB benefit design.
  • Explore innovative and blended financing options and private sector engagement models (e.g., performance-based financing, social contracting, corporate social responsibility, PPPs).
  • Propose strategies for resource mobilization, donor alignment, and transition planning, including clear roles for Global Fund, DOS/USG, and other partners.
  • Provide recommendations to improve efficiency, integration, and accountability of TB spending, including better use of existing funds, reduction of fragmentation, and stronger financial monitoring.

Drafting and Finalization of the TB Investment Plan

  • Consolidate findings from analyses and consultations into a draft TB Investment Plan that includes:
    • Vision and objectives through 2030.
    • Situation analysis and financing landscape.
    • Costing summary and funding scenarios.
    • Priority strategies and interventions by funding source.
    • Implementation, governance, and accountability arrangements.
    • Risk, transition, and sustainability considerations.
  • Develop clear, user-friendly presentation materials (PowerPoint and summary briefs) for senior decision-makers.
  • Present the draft TB Investment Plan to the DOH Task Force/TWG and other key bodies (e.g., TB NCC, PhilHealth, Global Fund Country Coordinating Mechanism, U.S. Government / DOS).
  • Facilitate the process to secure technical concurrence and policy endorsement from key agencies and bodies (e.g., TB NCC, DOH, PhilHealth, Global Fund, DOS/USG).
  • Incorporate feedback from validation processes and finalize the TB Investment Plan (full report and presentation).
  • Support DOH to outline next steps for operationalization, including integration with annual budgeting cycles, PhilHealth policy updates, LGU engagement, and partner alignment.

Deliverables. These shall be submitted within the 6-month period of engagement in accordance with the deadlines set by the DOH-NTP.

  • Inception Report (Workplan and Stakeholder Engagement Plan)
  • /Update Report in PowerPoint (following stakeholder consultations and validation workshops), including indicative amounts and possible program components per funding source
  • Draft TB Investment Plan (Vision to 2030 and key strategies/interventions), in PowerPoint format
  • Final TB Investment Plan (comprehensive report and PowerPoint presentation), incorporating consolidated feedback

REPORTING:

The Senior Technical Advisor will provide a detailed monthly report to the U.S. Department of State, submitted by the 5th day of the following month. The Advisor will also submit a weekly progress report to the U.S. Department of State (and participate in weekly calls) that includes a short paragraph on main successes and key issues for that week. The reporting format may be adjusted with concurrence from the COR.

Qualifications

  • Advanced degree in public health, health economics, health policy, or a closely related field.
  • At least 10 years of professional experience working with the Department of Health in the Philippines, the National TB Control Program, local governments, and development partners.
  • Demonstrated experience conducting or leading health financing analyses, health investment plans, funding requests/proposals, or costed health strategies, preferably as lead consultant or team lead.
  • Strong understanding of TB program design, implementation, and monitoring in high-burden settings.
  • Experience working with national health insurance schemes (preferably PhilHealth) and public financial management systems.
  • Excellent interpersonal skills and ability to work in a cross-cultural and multi-stakeholder environment.
  • Excellent written and oral communication skills in English.
  • Excellent written and oral communication in Filipino (Tagalog); knowledge of other Philippine languages is an advantage.
  • Strong computer skills, including proficiency with Microsoft Office (Word, Excel, PowerPoint).
  • Excellent organizational, planning, negotiation, and problem-solving skills.
  • Proven ability to prepare high-quality reports, presentations, and briefing papers under tight timelines.

Preferred Qualifications

  • Experience with U.S. Department of State and/or PEPFAR or other U.S. Government–supported projects is an advantage.
  • Experience engaging with the Global Fund, multilateral development banks (e.g., ADB, World Bank), or similar partners on TB or health financing.
  • Knowledge of Philippine health sector reforms, UHC implementation, and local government health systems.
  • Prior work on DOS- or U.S. Government–funded public health programs.
  • Relevant technical and regional experience in TB and health financing in the Philippines is preferred.
